Asbestos-Lead Accredited Training Program Reporting Forms

The Excel forms to be used to send notice of training programs and to report successful participants appear below.  Please choose the form you need and open the file by clicking on the appropriate spreadsheet link following the documentation.

Asbestos or Lead Training Notice


Data will be keyed into pre-formatted Excel spreadsheets. The necessary information is described below for each form. All data must be keyed in uppercase letters. Do not use any commas or periods. The date fields are formatted as mmddyyyy without slashes or dashes. The start time and end time columns on the notification form must be recorded as military time. 00:00 is midnight thru 23:59 which is 11:59 p.m. Use the pre-formatted spreadsheets sent to you as a template for entry. Leave the columns formatted as "text" columns. If you change their formatting values the data will not post through our programs. Files are to be saved from the spreadsheet as Save as Type: CSV (Comma delimited) (*.csv) to be e-mailed to notice@dpor.virginia.gov (see CSV instructions). The filename should be notice.csv for notifications.

Asbestos and Lead Training Notification:

1. Training Facility Name is the name of the facility where the course will be held. If there is no facility/building name, indicate the training provider name (ALL CAPS).
2. Training Facility Address Line 1 is the first line of address for the facility where the course will be held (ALL CAPS).
3. Training Facility Address Line 2 is the second line of address for the facility (room number, suite number, etc.) where the course will be held.
4. Training Facility City is the city location of the facility (ALL CAPS).
5. Training Facility State is the two digit code for the facility location. For example, enter VA for Virginia (ALL CAPS).
6. Training Facility Zip Code is the five or nine digit zip code for the facility. No slashes or dashes.
7. Training Start Date is the beginning date of the course. Date must be entered as mmddyyyy.
8. Training Start Time is the start time of the course. Entry must be in military time values such as 08:10 which is equal to 8:10 AM (colon must be used for this entry).
9. Training End Date is the ending date of the course. Date must be entered as mmddyyyy.
10. Training End Time is the end time of the course. Entry must be in military time values such as 16:30 which is equal to 4:30 PM (colon must be used for this entry).
11. Language of Course is the specific language the course was given in. Examples of entry are ENGLISH and SPANISH (ALL CAPS).
12. Course Accredited Number is the 10 digit DPOR approval number of the course. To confirm your appropriate number, you may go to the DPOR website License Lookup and enter your training provider name and indicate either Asbestos or Lead Training Provider Search.
13. Course Title is the type of course given. This entry will be either: WORKER, SUPERVISOR, INSPECTOR, MANAGEMENT PLANNER, PROJECT DESIGNER, PROJECT MONITOR or RISK ASSESSOR (ALL CAPS).
14. Initial or Refresher Indicator is either an I for Initial or an R for Refresher type course (ALL CAPS).
15. Contact Person for the course being given (ALL CAPS).  No periods, dashes or commas.
16. Contact E-Mail is the e-mail address of the contact person. This entry may include periods, lower case letters and any other symbol that is a part of the email address.
17. Contact Phone Number is the area code and phone number of the contact person. Do not use dashes/slashes. If preferred, you may include parenthesis for area code or, after the number, an extension.
18. Training Manager of the course (ALL CAPS).
19. Principal Instructor of the course. You may include several instructors, but do not include periods or commas to separate the listing. Allow several spaces between each name. The cell is programmed to wrap and will handle several listings.
20. The transaction indicator will be: O = Original Data being sent, R = Revision Data being sent, C = Cancel this course. Do not use the revision indicator to change dates of a course. You must cancel the original course. A new original notice must be sent with the new course dates.

Asbestos and Lead Training Program Participant List:

1. License # is the DPOR license number of the person taking the course. Leave this field blank if the person has not applied to DPOR for licensure or it had not been provided to the instructor.
2. Social Security Number or Virginia DMV Number of the person taking the course. DPOR must be notified (in writing) by the person taking the course if they want to substitute their Virginia DMV Number for their Social Security Number in their DPOR record.
3. First Name of Participant is the first name of the person taking the course (ALL CAPS).
4. Middle Initial of Participant is the middle initial of the person taking the course. Do not include the period punctuation (ALL CAPS).
5. Last Name of Participant is the last name of the person taking the course. (ALL CAPS). Do not use a hyphen if there are two or more last names. Only a space. This last name should be the same as the name(s) used on Virginia DMV or Social Security Number. Do not include "JR" or "SR" in this field.
6. Generation of Participant is the place to enter "JR" or "SR" or "III" for the person taking the course (ALL CAPS).
7. Participant Address Line 1 is the first line of residence address for the Participant (ALL CAPS).
8. Participant Address Line 2 is the second line of residence address for the Participant (ALL CAPS).
9. Participant City is the city location of the Participant (ALL CAPS).
10. Participant State is the two digit code for the Participant location. For example, enter VA for Virginia (ALL CAPS).
11. Participant Zip Code is the five or nine digit zip code for the Participant. No slashes or dashes.
12. Course Accredited # is the 10 digit DPOR approval number of the course that was taken by the Participant.
13. Course Name is the type of course given. This entry will be either: WORKER, SUPERVISOR, INSPECTOR, MANAGEMENT PLANNER, PROJECT DESIGNER, PROJECT MONITOR or RISK ASSESSOR (ALL CAPS).

18. Language of Course is the specific language the course was given in. Examples of entry are ENGLISH and SPANISH (ALL CAPS).
19. Exam Score is the passing grade given as a result of taking this course. This must be whole numbers and not using a percentage (%) sign.
20. Certificate Number Given at Course is the number assigned the Participant by the training provider. Do not use dashes, commas or periods. Spaces may be used.
21. The transaction indicator will be: O = Original data sent for this Participant´s results, R = Revision data sent for this Participant´s results.
