Important Notice for Applicants
Applicants for Licensure must read the Board regulations for the profession or occupation for which they are requesting licensure.

Application Format Information
One (1) format is provided: Adobe Reader 9.0 or later - Interactive PDF format.
The Adobe Interactive PDF allows you to type your information into the application and save a copy on your computer for your personal records. The customer is required to print, sign and return the application to our office at the address provided.
If you have Adobe Acrobat software installed on your PC, please make sure that you open the file using Adobe Reader. Some versions of Adobe Acrobat 8.0 or older will cause errors when using this application.
You will need to have version 9.0 or later of Adobe Reader installed on your computer. If you do not, then you will need to download the current Adobe Reader, which is free, before working with any PDF files. This is a onetime process which will then permit you to view and print these and other .PDF formatted publications on the Web.

DPOR Boards only accept the current versions of the applications provided below. DPOR Boards are not responsible for processing delays associated with forms that do not conform to this requirement. Application fees are not refundable.
