Application Format Information

You must have the Adobe Acrobat Reader in order to view and print the Adobe Acrobat PDF files, or you may use Microsoft Word version 7.0 or higher to open and edit (fill in) the MS Word Format forms. You can obtain the Adobe PDF Acrobat Reader or the Word Viewer from the Plug-ins page.

Rules for Accepting Applications

DPOR Boards only accept the current versions of the applications provided below. DPOR Boards are not responsible for processing delays associated with forms that do not conform to this requirement. Application fees are not refundable.

Course Requirements for Engineering Technology Program Form  Note:  This form is to be used by those applicants applying pursuant to regulation 18 VAC 10-20-210.4 with a degree in fields other than engineering or a related science, or with no degree.
License Renewal Form
Reinstatement Application Note: This form is only to be used to reinstate a license that has been expired for less than 5 years.  If the license has been expired for 5 years or more, you must use the License Application and the required attachments.
