Applications and Other Forms Related to Licensure
Important Information - PLEASE READ -
Each profession or occupation regulated by one of the Boards within the Virginia Department of Professional and Occupational Regulation requires an application for licensure or regulatory status, as well as the use of a variety of other forms.
Two form formats are provided: Microsoft Word, and Adobe Acrobat Reader PDF format.
NOTE: DPOR Boards will only accept application forms that print out exactly as the ones provided on this site. If the form does not print out on your printer as it appears on the screen, you should contact the appropriate Board office to request that a copy of the application form be sent to you through postal mail. DPOR Boards will not be responsible for delays resulting from use of forms printed from this web site (e.g. , request that a form be resubmitted on a proper form).
Microsoft Word Forms
The Microsoft Word license application versions can be filled in on your computer if you have the Microsoft Word 7.0 or later software version loaded on your computer; or the form can be printed and then filled in manually.
Adobe Acrobat Reader PDF Forms
The Adobe Acrobat Reader PDF version must be printed as a blank form before being filled in manually. The PDF version also requires the presence of the Adobe Acrobat Reader software before displaying the application.
In order to display and print the PDF version of the applications or forms, the Adobe Acrobat Reader software must be present on your system. If you do not have a copy of the Adobe's Acrobat Reader currently installed on your PC, you will need to download and configure your free copy before working with these files. This is a one time process which will then permit you to view and print these and other PDF formatted publications on the web. Please follow the installation and browser configuration instructions provided.
