Applicants for Licensure must read the Board regulations for the profession or occupation for which they are requesting licensure.

Application Format Information
You must have the Adobe Acrobat Reader in order to view and print the Adobe Acrobat PDF files or you may use Microsoft Word version 7.0 or higher to open and edit (fill in) the MS Word Format forms. You can obtain the Adobe PDF Acrobat Reader or the Word Viewer from the Plug-ins page.
Important Information about Acceptance of Applications
DPOR Boards will only accept application forms that print out exactly as the ones provided on this site. If the form does not print out on your printer as it appears on the screen, you should contact the appropriate Board office to request that a copy of the application form be sent to you through postal mail. DPOR Boards will not be responsible for delays resulting from use of forms printed from this Web site (e.g., request that a form be resubmitted on a proper form).
