LICENSE LOOKUP SEARCH INSTRUCTIONS

By License Number

If you know the ten-digit license number, no other entries are required.  If you know the last six-digits of the license number, please select the appropriate occupation and enter the six-digit license number.

By Name

This search will return a list of matching names from which you may choose more detailed information.  To begin your search, please choose at least one occupation along with entering the name.  A zip code may be entered as part of the search criteria along with the name. To limit the number of records returned, enter the most significant part of the name such as the last name.  If you know the first and last name, enter both.  The order in which the names are entered does not matter.  

Use an asterisk as a wildcard and to require the search to look for the first and last name in first name, last name order e.g. (JOHN *DOE).   If the search does not return a match with the first and last name, please try the last name and first initial followed by an asterisk e.g. (DOE J*).  An exact match on several words in a specific order may be made by including double quotes around the words.   If punctuation or special characters e.g. (&) are included as part of the search criteria, enclose the text in back quotes e.g. (`JONES & SMITH`); however, a search will work without including the special characters e.g.  (JONES SMITH).

By Zip Code

This search will return a list by matching on the zip code. Please choose at least one occupation. An entry in the name is not required.   Please enter the full zip code.  A link to the U.S. Postal Service is provided for detailed information on zip code areas.
